MEDICAL FORM

In order for your son to attend camp, we must have a completed medical questionnaire on file two weeks before the start of the season. Please note that this document includes an immunization record that must be completed by your son's physician. EQUIPMENT LIST

All boys attending Pine Island Camp must bring certain required items of clothing and equipment. Years of experience have taught us that these items are essential for participation in our hiking and canoeing trips, as well as for daily island living.
